The whole idea of therapy is to feel confident enough to get outside your comfort zone and take risks with your speech. Whatever the clinician can do to build the person's confidence and break the risk down into baby steps will help -- such as first asking the client to talk on the phone to a clinician in the same room. However, if a teen or adult client is so solidly stuck that they refuse to even try to use the phone, the only solution may be to discontinue therapy until the person is ready to change.
